Patient Information

On the day of your appointment:
If you are a new patient, please arrive 30 minutes prior to your scheduled appointment time. This is to allow adequate time to complete all necessary new patient paperwork.

All other patients please arrive 15 minutes prior to your scheduled appointment time. At Atlanta Pain and Spine Physicians, we take pride in having minimal wait times for our patients. In order to maintain this standard, it is necessary and greatly appreciated that our patients arrive on time for their scheduled appointments.

We do not accept walk-in appointments.

Please bring your photo I.D. and insurance card(s) to each appointment. This policy helps protect and ensure that no one may falsely receive your medical benefits. It also ensures accurate billing of your medical insurance.

Please be prepared to provide your medical history. This includes:

• A list of all medications that you are taking with the dosages and how you take each medication. This includes prescription medications, over-the-counter medications, and herbal medicine.
• Any relevant medical records from your referring physician
• Any relevant x-rays, imaging studies, and laboratory tests
• Details about your current condition.
• Your past medical history including surgeries and family history

For your convenience, you may download the New Patient Forms located on this page. This includes the medical history form, Privacy Act Notice, Consent for Treatment, and Medical Release forms. You may complete these at home and bring them with you on the day of your appointment. If you have questions regarding any of these forms, we will be happy to explain each one to you before you sign them.

• You will need a responsible adult to drive you home after your procedure and monitor you for the next 24 hours. Your driver must remain in the facility during your procedure.
• It is very important that you bring your complete medication list with you, including the dosages and the frequency of each medication you take.
• Do Not Eat or Drink 8 hours before your procedure.
• If you take blood pressure medication or medication for acid reflux, you make take these medications the morning of your procedure with just a sip of water.
• If you are a diabetic and you take oral diabetes medication or regular insulin, do not take your morning dose. If you take NPH, 70/30, or Ultralente insulin, take one-half your usual dose, and drink clear liquids (like black coffee or Sprite) up until 4 hours prior to your procedure.
• It is very important for you to notify the physician prior to your procedure if you are taking blood thinning medications. These include Aspirin, Plavix, Coumadin, Warafin, Pletal, Trental, and Aggrenox. These medications are generally stopped at least 7 days before a procedure. Please call our office for more specific instructions if you are taking one of these medications. It is very important that you always check with the prescribing doctor of your blood thinning medication before you stop taking or alter the dose of your blood thinning medication.
